The Hospice Interdisciplinary Team (IDT) members work together to provide coordinated care for the person with a life-limiting illness. The team includes doctors, nurses, social workers, grief specialists, hospice aides, clergy, therapists, etc., who will care for you. Each of these people offer support based on their special area of expertise. Together, they then give you and your loved ones complete palliative care aimed at relieving symptoms and providing social, emotional, and spiritial support.
The patient, family and caregivers are a very important part of the team because the plan of care is developed to meet the unique needs of each hospice patient and family. This is your journey-we are guides.
